Attach self certified copy of mark sheet & certificates (Original/Provisional) of all examinations passed.
Attach your Yoga experiences in a different sheet if needed.
The index card after affixing postages stamp of Rs.6/-and writing the full present address.
The medical history has been asked to help you in practical training. Please attach medical (diagnostic) documents if you have.
Attach the money receipt of the form.If form has been download form the website , then attached DD of Rs.300/- from any nationalised bank in favour of Indian Institute of Yogic Science & Research payble at Bhubaneswar.
